Onboarding Survey Improvements

Source: Call 34 (3-3), Call 35 (3-18). 3-18 has the most detailed requirements. Priority: Tier 2 — quick wins, high visibility for 3/25 visit KB Requirement IDs: #97-#109


Current State

6 questions: age, blood thinners, blood clot history, smoking, pets, someone helping after surgery. Timing: 2 weeks before surgery. Demonstrates with "New Nancy" scenario.

Opt-in engagement tracking (KB #106-#109)

Add a question offering activity/function baseline tracking:

Bawa: "Would you like to answer a few questions about your current level of activity or function and we can then repeat these throughout your rehab to chart your progress?"

  • If yes → system captures baseline and repeats periodically
  • If no → system skips, does not re-prompt
  • "Everybody may have different appetites for their level of engagement"
  • Tailoring applies to both patients AND providers

Scope control (KB #111)

Bawa: "I don't know if we need to include all of those things. I think that's a little too much."

Context: there's a lot of pre-op checklist items in literature (nails, sheets, dentist). Don't include all of them. Keep onboarding focused on things that change the system's behavior.


Design Principles

Self-service only (KB #99)

Bawa: "The person signing up... they're gonna have to put in their data themselves... we want to eliminate the process of where it's getting input from the office."

No office staff dependency. No external system integration for data entry. Patient does everything.

Pre-surgery timing confirmed (KB #105)

Lauryn: "These questions are going to be asked prior to... this is before surgery even happens."

Current 2-weeks-before timing is appropriate.


From 3-3 Call

Onboarding as protocol customizer

Blood thinner answer eliminates unnecessary DVT/aspirin reminders. This pattern extends to all new questions: - Procedure type → different protocol content - Surgery side → different activity restrictions - Engagement opt-in → different check-in frequency

Protocol frequency driven by onboarding

Bawa (3-3): Don't message every day — too aggressive. Use onboarding info to customize timing.

Example: wound dressing reminders days 5-8 but bandage doesn't come off until day 8 = 3 days of pointless reminders. Onboarding + goals should customize protocol timing.
